Studies show that patients who are referred to hospice care as early as possible experience a far superior quality of life, with fewer hospitalizations and a decrease in symptoms.
What is hospice? It is an interdisciplinary team that includes caregivers, nurses, social workers and chaplains. We visit you, wherever you are living—at home, in a nursing home or at any of the hospitals in Dutchess and Ulster counties—and work in tandem with your current physician to provide an extra level of specialized comfort.
Referrals for hospice care can come from you, your loved ones, your own doctors, or a hospital. Medicare-certified hospices require physicians to certify that a patient's prognosis is expected to be six months or less.
At Hudson Valley Hospice, we know that each patient’s situation is unique, so we develop an individualized plan of care to provide the services that meet your specific needs. Your care plan is continually reviewed and updated by our medical director, your physician, and members of your clinical care team. Some of the services provided by the clinical care team are:
When a patient reaches the end stages of an illness or disease, hospice offers palliative care to help control pain and reduce anxiety. We improve quality of life and time spent with loved ones.
Emotional and Spiritual Support
Hospice care provides emotional and spiritual support to patients and families, including grief and bereavement resources. You and your loved ones don’t have to face these challenges alone.
Medical Care Management
Hospice manages medical care, in-home visits from care providers, acquiring medical supplies, and end-of-life planning.
Respect for the Patient and Family Wishes
Hospice strives to provide the highest quality end-of-life care so that our patients may live with comfort and dignity. Every family is on a different journey, and we acknowledge and respect your decisions and plans. Our goal is to make this difficult time as easy as possible.
We improve quality of life by relieving patients and their loved ones of much of the stress that comes with providing end-of-life care. Families can focus on being together and sharing memories.
